AUTHORS ABSTRACTEine Allergie gegen Penizilline oder Cephalosporine äußert sich entweder innerhalb weniger Minuten nach Einnahme beziehungsweise Infusion mit akuten Anaphylaxiesymptomen oder mehrere Stunden bis Tage später als exanthematische Hautreaktion. Am häufigsten werden zurzeit Arzneimittelexantheme durch Amoxicillin oder Ampicillin diagnostiziert. Als Auslöser für β;-Laktamantibiotika-induzierte IgE-vermittelte anaphylaktische Reaktionen sind bestimmte Cephalosporine wie Cefazolin, Ceftriaxon oder Cefuroxim von zunehmender Bedeutung. Diese Arbeit ist ein Überblick ausgewählter, wissenschaftlicher Artikel und wurde mit Recherchen in Pubmed, Studien und Fachbüchern erstellt. Die Art und Weise der allergologischen Diagnostik bei Penizillinallergikern orientiert sich an anamnestischen Angaben und ärztlicher Befunddokumentation; routinemäßig werden IgE-Messungen, Haut- und Provokationstestungen angewandt. Während die Bestimmung von IgE-Antikörpern gegen bestimmte Penizillindeterminanten zu den wenigen aussagekräftigen Laboruntersuchungen in der Diagnostik von Arzneimittelreaktionen gehört, bleiben Spezialmethoden wie der Basophilenaktivierungstest oder der Lymphozytentransformationstest erfahrenen Labors vorbehalten, die ihre Ergebnisse entsprechend kritisch bewerten. Quaddeln mit Umgebungsrötungen in Prick- und Intrakutantests sprechen für IgE-vermittelte Allergien, infiltrierte Erythem- oder Ekzemplaques in Epikutan- oder Intrakutantests für exanthematische Spättypallergien. Der prädiktive Wert einer kombinierten allergologischen In-vitro- und In-vivo-Diagnostik mit β-Laktamantibiotika liegt bei mehr als 90 %.
