ABSTRACT

Background.The study aims to investigate diffusion, typology, pre-preanalytical (clinical guidelines and objectives), pre-analytical (matrix), analytical (internal and external quality control, Turn Around Time (TAT)) and post-analytical (measurement units, purposes) characteristics of natriuretic peptides (CNP) in Italy.Methods.The data were collected through the IV survey on the state of the art of cardiac markers in Italy conducted in 2017 by the Study Group of Myocardial Markers of the Italian Society of Laboratory Medicine and Clinical Pathology (GdS MM SIPMeL). The questionnaire, accessible on the SurveyMonkey® platform (from May 15, 2017 to December 15, 2017) and available on the Society’s website at the following address: http://www.sipmel.it/it/comunicazione/notizie/109706, contained 22 items on the use of CNP.Results.122 questionnaires of 152 collected were validated. 75% of total respondents declare to determine CNP (47% only NT-proBNP, 44% only BNP, 9% both). The main users are emergency and cardiology departments and, with a significant increase compared to what was observed in 2012, the Medical Area. The majority of respondents (78%) provide CNP both urgently and routinely. The most frequent use (50–90%) of CNP is for diagnosis (acute dyspnea/heart failure and/or monitoring heart failure and/or prognosis) while only the minority (27%) use it also for the therapy guide. A clear improvement is observed compared to the previous 2012 survey on the correct use of matrices (BNP: 95.5% EDTA; NT-proBNP: 89% serum and heparin plasma). There is still a significant percentage of respondents who do not perform Internal Quality Control (IQC) (4%) and External Quality Assessment (EQA) (15%). The analytical methods used are: BNP: Abbott Architect 27.2%, Alere Triage 5.5% (alone), Beckman 42.2% (often integrated by Alere), Siemens Centaur 22.7%, others 2.4%; NT-proBNP: Alere Triage 2.4% alone, AQT90 Radiometer 5%, VIDAS Biomerieux 15%, Roche 57% (prevalently with ECLIA method), Siemens 15%, others 2.4%. The response rate of the TAT question is 81%: 64% of respondents measure it from the sampling to the report and 56% from receiving the sample to the report; in both cases, the majority reports within 60 minutes. The unit of measurement used is in 99% of the cases pg/ml and in 1% pmol/L. 85% of respondents answer the questions on the decisional levels (LD), bringing out a significant dispersion (absolute value of the LD, single/multiple, differentiated by age, distinct for acute/non-acute patients).Conclusions.Compared to the 2012 survey, there has not been an increase in CNP users despite their use being recommended for diagnosis/prognosis in national/international guidelines. The ratio between BNP and NT-proBNP users is maintained while an increase in Laboratories measuring both molecules is observed. The number of users who choose to assay CNP (also) with Point-of-Care-Testing (POCT) is also increasing, probably contributing further to the difficulties in comparing the results. The quality of the whole process, from the pre-analytical to the post-analytical phase, has improved compared to 2012, although there remains a great variability in the choices of the LD, probably fed by the poorly harmonized recommendations of the international guidelines which, among other things, do not mention the fact that LDs can be significantly influenced by analytical inter-method differences.     10 schema:description Background.The study aims to investigate diffusion, typology, pre-preanalytical (clinical guidelines and objectives), pre-analytical (matrix), analytical (internal and external quality control, Turn Around Time (TAT)) and post-analytical (measurement units, purposes) characteristics of natriuretic peptides (CNP) in Italy.Methods.The data were collected through the IV survey on the state of the art of cardiac markers in Italy conducted in 2017 by the Study Group of Myocardial Markers of the Italian Society of Laboratory Medicine and Clinical Pathology (GdS MM SIPMeL). The questionnaire, accessible on the SurveyMonkey® platform (from May 15, 2017 to December 15, 2017) and available on the Society’s website at the following address: http://www.sipmel.it/it/comunicazione/notizie/109706, contained 22 items on the use of CNP.Results.122 questionnaires of 152 collected were validated. 75% of total respondents declare to determine CNP (47% only NT-proBNP, 44% only BNP, 9% both). The main users are emergency and cardiology departments and, with a significant increase compared to what was observed in 2012, the Medical Area. The majority of respondents (78%) provide CNP both urgently and routinely. The most frequent use (50–90%) of CNP is for diagnosis (acute dyspnea/heart failure and/or monitoring heart failure and/or prognosis) while only the minority (27%) use it also for the therapy guide. A clear improvement is observed compared to the previous 2012 survey on the correct use of matrices (BNP: 95.5% EDTA; NT-proBNP: 89% serum and heparin plasma). There is still a significant percentage of respondents who do not perform Internal Quality Control (IQC) (4%) and External Quality Assessment (EQA) (15%). The analytical methods used are: BNP: Abbott Architect 27.2%, Alere Triage 5.5% (alone), Beckman 42.2% (often integrated by Alere), Siemens Centaur 22.7%, others 2.4%; NT-proBNP: Alere Triage 2.4% alone, AQT90 Radiometer 5%, VIDAS Biomerieux 15%, Roche 57% (prevalently with ECLIA method), Siemens 15%, others 2.4%. The response rate of the TAT question is 81%: 64% of respondents measure it from the sampling to the report and 56% from receiving the sample to the report; in both cases, the majority reports within 60 minutes. The unit of measurement used is in 99% of the cases pg/ml and in 1% pmol/L. 85% of respondents answer the questions on the decisional levels (LD), bringing out a significant dispersion (absolute value of the LD, single/multiple, differentiated by age, distinct for acute/non-acute patients).Conclusions.Compared to the 2012 survey, there has not been an increase in CNP users despite their use being recommended for diagnosis/prognosis in national/international guidelines. The ratio between BNP and NT-proBNP users is maintained while an increase in Laboratories measuring both molecules is observed. The number of users who choose to assay CNP (also) with Point-of-Care-Testing (POCT) is also increasing, probably contributing further to the difficulties in comparing the results. The quality of the whole process, from the pre-analytical to the post-analytical phase, has improved compared to 2012, although there remains a great variability in the choices of the LD, probably fed by the poorly harmonized recommendations of the international guidelines which, among other things, do not mention the fact that LDs can be significantly influenced by analytical inter-method differences.
