ABSTRACT

Pheochromocytoma is a catecholamine-producing neuroendocrine tumor derived from the neural crest. It is a rare tumor with an incidence in the general population of 2–8 cases per million population per year. Pheochromocytomas most commonly arise in the adrenal medulla but they can also occur extra-adrenally; these tumors are referred to as paragangliomas. The clinical presentation involves a constellation of signs and symptoms associated with catecholamine excess: hypertension (paroxysmal or sustained), sweating, pallor, headaches, palpitations, anxiety, dyspeptic complaints, paresthesias, visual disturbances, chest and abdominal pain.The use of plasma free metanephrines or urinary fractionated metanephrines, determined by liquid chromatography with electrochemical detection or liquid chromatography with mass spectrometry, has been recommended as a first-line diagnostic test in the recent practice guidelines proposed by the Endocrine Society; in fact they have a high sensitivity, allowing to exclude a pheochromocytoma in the presence of a negative result, while the same conclusion can not be reached by determination of catecholamines or their urinary metabolite, vanilmandelic acid. Moreover, Endocrine Society guidelines suggest drawing blood with the patient in the supine position for 30 minutes and the use of reference intervals established in the same condition.For borderline concentrations of metanephrines, the measurement of plasma chromogranin A is recommended and in the few cases, when the determination of plasmatic metanephrines, urinary fractionated metanephrines and plasma chromogranin A are not sufficient for pheochromocytoma diagnosis, clonidine suppression test is suggested.Urinary vanilmandelic acid and plasma and urinary catecholamines determination should be discontinued.
