ABSTRACT

L’épidémiologie bactérienne est caractérisée par une augmentation constante de la résistance aux antibiotiques. Les bactéries hautement résistantes émergentes (BHRe) sont définies comme des bactéries commensales du tube digestif à fort pouvoir épidémiogène. Ces bactéries présentent des mécanismes de résistance encore peu implantés en France, mais déjà endémiques dans les hôpitaux de nombreux pays. Il s’agit des entérocoques résistants à la vancomycine (ERV) et des entérobactéries productrices de carbapénémases (EPC). Vis-à-vis de ces souches multirésistantes, l’arsenal thérapeutique est extrêmement restreint, en particulier en pédiatrie, nécessitant l’administration de combinaisons d’antibiotiques dont l’utilisation chez l’enfant est encore peu documentée. De façon à éviter la diffusion de ces souches dans notre système de santé et à préserver l’activité de certains antibiotiques comme les carbapénèmes, des mesures drastiques d’isolement doivent être instaurées dès l’admission d’un patient à risque (patient ayant des antécédents d’hospitalisation à l’étranger dans l’année précédente, rapatrié sanitaire de l’étranger) et bien sûr en cas de découverte d’un patient porteur en cours d’hospitalisation.
