ABSTRACT

IntroductionInflammatory myofibroblastic pseudotumors are rare benign lesions of the soft tissue. The pancreas is a very rare location of these lesions. We present a new case of pancreatic inflammatory myofibroblastic pseudotumor with literature review.Case reportA 30-year-old woman with no specific medical history consulted for epigastric pain, developed since 3 months. Slight epigastric tenderness was observed on clinical examination. The imaging results showed a pancreatic hypodense mass with infiltration of the peripancreatic fat without metastasis or peritoneal carcinosis. Surgical exploration showed a nonresectable pancreatic mass invading the mesentery and the transverse mesocolon. Pathological study showed a chronic pancreatitis without any signs of malignancy. Two other echoguided transparietal biopsies of the mass were also without malignancies. The approach adopted was to follow-up with no treatment, and the outcome was marked by the disappearance of the mass after a year. Currently, there is no recurrence after 4 years of follow-up.CommentsTo date, 25 cases of pancreatic inflammatory myofibroblastic pseudotumors have been reported. These lesions are a significant diagnostic problem with the pancreatic adenocarcinoma, and often the pathological study of the pancreatectomy specimen is the only way for diagnosis. Their prognosis seems good with the possibility of local recurrence and a rare case of malign transformation.ConclusionThe pancreatic inflammatory myofibroblastic pseudotumors are a rare entity, which can mimic clinically and radiologically a pancreatic adénocarcinoma. The special feature of our observation is the spontaneous disappearance of the lesion without treatment despite the initial invasive appearance. More... »
