ABSTRACT

La relation soignant–soigné est asymétrique. Elle repose sur la demande du patient, étymologiquement « celui qui endure », demande en direction du soignant supposé savoir comment le soulager. Dans cet article, nous interrogeons cette asymétrie initiale autour de la douleur. Nous nous intéressons au masochisme chez chacun des partenaires en regard de l’autre, à la manière dont se décline la relation soignant–soigné entre douleur subie et douleur consentie. Du côté du soigné, le masochisme permet de supporter une position de passivité face à la douleur et face aux soins. Parfois, une dose de douleur contrôlée peut être recherchée en réaction de défense contre cette passivité. Une part de masochisme est également attendue du côté des professionnels qui travaillent en structure douleur. En effet, c’est une clinique complexe nécessitant de se départir d’une maîtrise complète de la situation au profit de l’approche pluriprofessionnelle. Rendre le patient douloureux chronique acteur de sa prise en charge suppose également une certaine retenue du côté des soignants. Enfin, être confronté quotidiennement à la douleur engage le masochisme comme condition de l’empathie. Il nous apparaît important de reconnaître la part masochiste normale chez le patient comme chez le soignant et d’envisager également chez chacun d’entre eux les risques de dérives et d’excès qui peuvent émerger de la relation de soin.
