ABSTRACT

Fibromyalgia is a poorly understood pain disorder. Biological, psychological and social factors likely contribute to its pathogenesis. Diffuse pain, clinical signs, such as allodynia and hyperalgesia, the efficacy of adjuvant analgesics, and experimental data point to central sensitization as a key factor in the pathophysiology of fibromyalgia. Central sensitization could be explained by increased input from the peripheral nervous system, but growing evidence emphasizes the role of central facilitation. Moreover, genetic factors in pain sensitivity are currently under study. Although fibromyalgia should not be considered a classical neuropathic pain syndrome, central sensitization is now recognized as a major factor in its pathogenesis. More... »
