ABSTRACT

Die Versorgung von proximalen Humerusfrakturen gewinnt in den letzten Jahren eine immer größere Bedeutung, da mit zunehmendem Alter der Patienten auch die Anzahl dieser osteoporoseassoziierten Frakturen steigt. In dieser Studie wurden der minimal-invasive Deltasplitzugang (DS) und der deltoideopektoralen Zugang (DP) bei der operativen Versorgung proximaler Humerusfrakturen mit der polyaxialen, winkelstabilen NCB- (Non-contact-bridging-)Platte verglichen. In diese randomisierte Studie wurden 120 Patienten aufgenommen. Neben der Erfassung relevanter Behandlungsparameter (Operationszeit, Durchleuchtungszeit, Verweildauer) wurden die Patienten bei Entlassung, 6 Wochen und 6 Monate postoperativ klinisch (Constant-Score, VAS für Schmerz, ADL) und radiologisch untersucht. Alter des Patientenkollektivs, Operationszeit, Durchleuchtungszeit und Komplikationsraten waren vergleichbar. Hinsichtlich der klinischen Untersuchung nach 6 Monaten lässt sich im Constant-Score ein tendenzieller Vorteil für den minimal-invasiven DS-Zugang erkennen. Die NCB-PH-Platte kann auch über den minimal-invasiven Deltasplitzugang sicher implantiert werden. Der Zugang hat möglicherweise einen Einfluss auf das klinische Outcome. More... »
