ABSTRACT

Present work aims at sensitizing the failure analysts about the importance of visiting the site of failure and careful visual observations of the failed components as well as the system in totality. The crucial role of visual observations is illustrated using two case studies on failures of (i) expansion bellow of gas cleaning system of an iron blast furnace and (ii) set of work roll and back-up roll. In the former case, visual observations alone could help in arriving at the most probable cause of failure and taking corrective actions that prevented any subsequent failure. In case of failure of rolls, the chronology of failure could be established. It was concluded from the visual observation itself that the fatigue failure of the work roll was followed by catastrophic brittle fracture of the back-up roll. Thus, visual observations can provide important guidelines for subsequent in-depth metallurgical analysis to delve deeper into the mechanism of failures. More... »
