ABSTRACTChronic myelomonocytic leukemias (CMML) are clonal stem cell disorders characterized by persistent monocytosis in the peripheral blood. The classification into CMML‑0, CMML‑I, and CMML-II depends on peripheral and bone marrow blast counts. Furthermore, differentiation into myelodysplastic or myeloproliferative CMML depends on white blood cell counts (≥13,000/µl). Compared to the myelodysplastic variant, myeloproliferative CMML is associated with a worse outcome. Of all patients, 70% present with a normal karyotype, whereas somatic mutations occur in 90% of all cases and may have prognostic impact. For prognostic assessment, the CMML Prognostic Scoring System (CPSS) and the CPSS incorporating molecular parameters (CPSS-mol) are recommended. Allogeneic hematopoietic stem cell transplantation is the only curative treatment approach. The only approved treatment for myelodysplastic CMML with more than 10% bone marrow blasts is 5‑azacytidine. Treatment of CMML patients within clinical trials is highly recommended. More... »
