ABSTRACTLes simulateurs de chirurgie ont eu un essor récent important. Le but de notre travail était de créer un modèle pelvien spécifique à chaque patient qui servira de base au développement d’un simulateur patient-spécifique. La reconstruction tridimensionnelle des organes pelviens a été réalisée à partir d’images IRM de 15 patientes sans trouble de la statique pelvi-périnéale. Les organes pelviens étaient segmentés par méthode semiautomatique, puis leur maillage était réalisé. Les paramètres mécaniques et physiques de chaque organe étaient définis. La modélisation des organes pelviens était intégrée au sein d’un modèle de déformation physique. Les contraintes simulant les poussées abdominale et diaphragmatique étaient exercées sur le modèle. Les déformations étaient évaluées en recalant les images de la simulation et les images de l’IRM dynamique. La durée d’acquisition des images IRM nécessaires à la modélisation était inférieure à sept minutes. La morphologie des organes pelviens après segmentation était spécifique à chaque acquisition. La modélisation dynamique a été possible dans tous les cas. La concordance entre les images IRM et les images de la modélisation était excellente au repos et satisfaisante lors des efforts de poussée. Les principales variations de volume concernaient la vessie. Ce travail a permis la création d’un modèle spécifique statique et dynamique de pelvis féminin. Il est la base indispensable à l’élaboration d’un simulateur de chirurgie pelvienne.
