ABSTRACT

ObjectiveThe purpose of this study was to compare the smooth muscle content of the anterior vaginal wall in women with pelvic organ prolapse (POP) and in control subjects. A second analysis was performed to compare women with recurrent POP and primary cases.Material and methodsSpecimens were taken from the apex of the anterior vaginal cuff from 11 women with POP and from eight control subjects who had undergone a hysterectomy without prolapse. The second analysis compared six women with primary POP and five women with recurrent POP. Masson’s trichrome stain was used to determine the distribution of collagen in the extracellular matrix of the vaginal muscularis and quantify the collagen in the area of interest. Immunochemistry was used to incubate slides of alpha smooth muscle actinantibodies. Morphometric analysis was used to compare and quantify the smooth muscle content of the vaginal muscularis in control subjects and women with genital prolapse.ResultsThe fractional area of nonvascular vaginal smooth muscle in women with POP was significantly decreased compared to control subjects (41.9 vs 61.9%, P = 0.0005 [95% CI: 10.8–28.9]). The fractional area of connective tissue was significantly increased (56.8 vs 35%, P = 0.0002 [95% CI: 12.9–30.8]). The fractional area of blood vessels was similar (2.2 vs 3.4%, P = 0.09 [95% CI: −2.8–0.3]). Results of the second analysis showed no significant difference in fractional area of nonvascular vaginal smooth muscle between primary cases and recurrent POP (40 vs 44%, P = 0.62 [95% CI: −22.7–14.33]). The fractional area of connective tissue showed no significant difference (59.6 vs 53.4%, P = 0.46 [95% CI: −12.1–24.4]). There was no significant difference in fractional area of blood vessels (2.1 vs 2.3%, P = 0.69 [95% CI: −1.6–1.1]).ConclusionsThe fraction of smooth muscle in the muscularis of the anterior vaginal wall was significantly decreased in women with POP compared with normal control subjects. There was no significant difference between primary and recurrent cases. Reversibility of these histopathological findings may influence success rates of surgical treatment using biomaterials.
