ABSTRACT

Les troubles ano-rectaux au cours des accidents vasculaires cérébraux sont fréquents, mais les études épidémiologiques prospectives sont rares. Ces troubles sont dominés par l’incontinence fécale et la constipation. La constipation est le plus souvent multifactorielle, d’étiopathogénie non spécifique (sujet alité, médications anticholinergiques, régime peu adapté) ou directement secondaire à l’AVC (paralysie motrice recto-anale, troubles sensitifs anorectaux, asynchronisme abdomino-pelvien). Son traitement est toujours nécessaire (régime, mucilages, suppositoires exonérateurs par distension volumétrique rectale, lavements évacuateurs, évacuation de fécalome) en raison des risques potentiels (incontinence fécale de regorgement, décompensation ou pérennisation de troubles vésico-sphinctériens). L’incontinence anale procède elle aussi de causes iatrogènes (sédatifs, fécalome...), mais aussi de lésions secondaires à l’AVC (paralysie sphinctérienne, troubles de la conscience initiaux, troubles psychocomportementaux séquellaires).
