ABSTRACTEin kleiner Prozentsatz von Patienten mit Zöliakie leidet trotz einer strikt glutenfreien Diät weiterhin an einem Malabsorptionssyndrom bei zöliakietypischer intestinaler Zottenatrophie. Dieses Krankheitsbild wird als refraktäre Zöliakie bezeichnet. Sie wird auf Basis der Immunhistologie sowie der Klonalität des T-Zell-Rezeptor-Gens weiter in eine refraktäre Zöliakie Typ I mit autoimmunem Charakter bzw. in eine refraktäre Zöliakie Typ II mit Prälymphomcharakter klassifiziert. Hiervon klar zu trennen sind sog. Spruesyndrome, denen die typischen Charakteristiken einer Zöliakie bis auf die morphologischen Veränderungen der Dünndarmschleimhaut gänzlich oder z. T. fehlen. Die refraktäre Zöliakie Typ I wird in der Regel bei Symptomen immunsuppressiv behandelt, während sich dieses Vorgehen bei Nachweis einer refraktären Zöliakie Typ II verbietet. Das Risiko, dass sich aus der refraktären Zöliakie Typ II ein enteropathieassoziiertes T-Zell-Lymphom (ETZL) entwickelt, ist massiv erhöht, sodass hier eine ausgiebigere Diagnostik inklusive einer Schnittbildgebung und ggf. Knochenmarkspunktion zu erfolgen hat. Bei vorhandenen Symptomen sollte die refraktäre Zöliakie Typ II eher aggressiv im Sinne einer Lymphomtherapie behandelt werden.
