ABSTRACT

Die autoimmune Enteropathie (AIE) ist eine seltene Erkrankung des Darms, die üblicherweise Kinder betrifft. In den letzten Jahren häufen sich jedoch Publikationen zum Vorkommen bei Erwachsenen. Klinisch zeigt sich eine Diarrhö mit Gewichtsverlust bei Malabsorption. Diagnostisch findet sich ein zöliakieähnliches Bild mit Zottenatrophie, Kryptenhyperplasie und lymphozytärer Infiltration der Lamina propria. Im Gegensatz zur Zöliakie findet sich nicht die typische Vermehrung der intraepithelialien Lymphozyten. Antienterozytäre Antikörper sowie Antikörper gegen Becherzellen („anti-goblet cell antibodies“, AGCA) stellen einen wichtigen Bestandteil der Diagnose dar, da entsprechende Untersuchungen in ca. 90% der Fälle positiv ausfallen. Es finden sich gehäuft andere autoimmune Erkrankungen. Therapeutisch benötigen die Patienten neben einer intensiven supportiven Therapie eine Induktionstherapie mit Steroiden und in der Regel eine langjährige Immunsuppression mit Steroiden, Cyclosporin A, Tacrolimus, Cyclophosphamid oder Infliximab.
