ABSTRACT

Epilepsiechirurgie gilt bei Temporallappenepilepsien (TLE) mit Hippocampussklerose (HS) seit Jahrzenten als Therapie der Wahl. Es sind verschiedene chirurgische Methoden etabliert, um die pathologisch veränderte Region des mesialen Temporallappens inklusive der Hippocampusformation zu resezieren. Eine Studie mit homogenen Patientengruppen zum Vergleich der operativen Techniken und zur Analyse der besten Methodik wurde nach unserem besten Wissen bisher methodisch nicht so durchgeführt, wie dies in unseren beiden preisgekrönten Originalarbeiten beschrieben wurde. Ziel der beiden Arbeiten war ein Vergleich der selektiven Amygdalohippocampectomie mit der klassischen 2/3-Temporallappenresektion im Hinblick auf die postoperative Anfallsfreiheit und das kognitive Outcome in homogenen Patientengruppen mit therapierefraktärer TLE und unilateraler Hippocampussklerose. Die Ergebnisse belegen keine eindeutig zu favorisierende Methode, werden aber hinsichtlich der prognostischen Beratung individualspezifisch hilfreich sein. More... »
