ABSTRACT

Neben der rein visuellen EEG-Befundung stellt die Quellenanalyse eine ergänzende Methode zur Lokalisationsdiagnostik epilepsietypischer Aktivität dar. Unter dem Vorwärtsproblem versteht man die Frage, wie ein elektromagnetisches Feld an der Oberfläche bei einem gegebenen intrazerebralen Generator verteilt wäre. Im Gegensatz zu magnetischen Feldern werden elektrische Felder stark durch die Leitfähigkeiten der Gewebe im Volumenleiter verzerrt. Daher sind sie stärker vom zugrunde gelegten Kopfmodell abhängig. Für das „inverse Problem“, also die Lokalisation der Quelle eines gegebenen Oberflächenfelds, gibt es keine eindeutige Lösung. Quellenmodelle, deren an der Oberfläche zu erwartendes elektromagnetisches Feld möglichst gut mit den gemessenen Daten übereinstimmt, können als näherungsweise, „äquivalente“ Lösung dienen. Zur Lokalisation des Zentrums einer kortikalen Aktivierung können verschiedene diskrete oder verteilte Quellenmodelle angewandt werden, die jeweils unterschiedliche Vor- und Nachteile haben. Allerdings kann keines der Modelle die kortikale Ausdehnung sicher bestimmen. Eine zufriedenstellende Lokalisationsgenauigkeit und der klinische Nutzen der Quellenanalyse konnten im Rahmen der prächirurgischen Epilepsiediagnostik gezeigt werden. More... »

Quellenmodelle, deren an der Oberfläche zu erwartendes elektromagnetisches Feld möglichst gut mit den gemessenen Daten übereinstimmt, können als näherungsweise, „äquivalente“ Lösung dienen. Zur Lokalisation des Zentrums einer kortikalen Aktivierung können verschiedene diskrete oder verteilte Quellenmodelle angewandt werden, die jeweils unterschiedliche Vor- und Nachteile haben. Allerdings kann keines der Modelle die kortikale Ausdehnung sicher bestimmen. Eine zufriedenstellende Lokalisationsgenauigkeit und der klinische Nutzen der Quellenanalyse konnten im Rahmen der prächirurgischen Epilepsiediagnostik gezeigt werden.
