ABSTRACT

The increasing prevalence of overweight and obesity worldwide increasingly affects women in childbearing age. Undesired or unplanned pregnancies occur more frequently in overweight women, which can be attributed to the inadequate use of an effective contraception and to the pathophysiological characteristics associated with an increased proportion of body fat. Therefore, an adequate contraception counselling of obese women is essential for the health and quality of life. Among the combined contraceptives, oral preparations that contain ethinyl estradiol and levonorgestrel represent an efficient option, provided that in addition to an increased body mass index (BMI) there are no further risk factors for cardiovascular or thromboembolic events. Pure gestagen preparations are characterized by a low risk to health. The fear of a further increase in weight is a frequent reason for stopping the intake, whereby for most preparations this association has not been scientifically confirmed, except for depot medroxyprogesterone acetate. The effectiveness of intrauterine contraceptives develops independently from body weight. The insertion of a copper-containing intrauterine pessary is the most certain method for emergency contraception but is substantially more complicated and expensive than emergency oral contraceptives. The oral intake of 30mg ulipristal acetate is preferred to 1.5mg levonorgestral due to the more potent inhibition of ovulation and the stronger reduction of unwanted pregnancies. More... »
