ABSTRACT

Depuis une dizaine d’années, l’informatisation des outils d’imagerie et de traitement a radicalement transformé les procédures de radiothérapie. Les améliorations ont porté sur tous les aspects techniques et méthodologiques de l’irradiation thoracique: l’immobilisation, l’imagerie tridimensionnelle, la définition des volumes cibles et des organes à risque, la simulation virtuelle, le calcul tridimensionnel, la balistique, le repositionnement quotidien et le contrôle de la respiration. Il n’y a désormais plus de comparaison possible avec la radiothérapie bidimensionnelle basée sur une préparation radiologique conventionnelle. Certaines des innovations retenues sont spécifiques à la topographie du thorax et à la fonction respiratoire qui rendent la radiothérapie du poumon très complexe. Les améliorations ainsi obtenues permettent assurément aujourd’hui d’optimiser la qualité des traitements et d’envisager des protocoles personnalisés. More... »
