ABSTRACT

L’échoendoscopie linéaire (EUS), utilisant le doppler pulsé ou le doppler couleur, est une technique prometteuse, et de nombreuses études décrivent des aspects spécifiques dans l’hypertension portale. L’EUS est une technique fiable pour le diagnostic de thrombose porte, il a l’avantage de ne pas utiliser de produit de contraste chez les patients cirrhotiques. L’EUS donne une vision directe des vaisseaux, permettant une analyse détaillée (l’épaisseur des parois vasculaires, les thromboses partielles ou totales, le flux dans la veine porte et les collatérales périgastriques). Le débit azygos, reflet de la circulation collatérale et des varices, peut être étudié en EUS. De nombreuses techniques interventionnelles peuvent être développées grâce à l’EUS (oblitérer des varices et diagnostiquer un hépatocarcinome).
