ABSTRACT

There are clinical, functional and theoretical aspects important to decision making in terms of the indication for corrective osteotomies in the lower leg. The indication is based not only on biomechanical evaluations, but also on the patient’s level of discomfort and the potential benefit to be gained from surgery. Measurements exceeding tolerable, empirically defined axis deviations support the indication. Results of X-ray analysis compared to the contralateral side are necessary for the indication and perioperative planning. Instability of knee ligaments must be taken into account when planning a metaphyseal osteotomy in that region. Fixed-angle plate systems are well suited for osteosynthetic procedures in the knee area and supramalleolar. Diaphyseal axis deviations are predominantly treated by means of intramedullary nails. Unreamed nails alone are not suitable for rotational correction, and should be combined with an additional plate. External fixation may be useful in particular situations, especially for stepwise corrections and lengthening, as well as extensive trauma-related soft tissue damage.
