ABSTRACT

High-speed trauma is the most common cause of fractures of the distal femur and the tibial head. Although periprosthetic distal femoral fractures are occurring with increasing frequency in the elderly, fractures of the tibial head are the most common injuries. Ligament and meniscal injuries of the knee are frequently combined with these fractures, but in many cases they are missed in the initial diagnosis. Operative treatment should be preferred because precise and anatomical repositioning is a prerequisite for prevention of posttraumatic arthritis. Another advantage of surgery is the early postoperative functional therapy; the long-term fixation in a cast that is necessary in the case of conservative treatment may result in a poor functional outcome. With concomitant soft tissue injuries and in multiply injured patients these fractures should initially be fixed in an external fixator, followed by elective internal stabilisation. Arthroscopy, angle-stable plating, intramedullary stabilisation and LISS (less invasive stabilisation system) osteosynthesis can be used for definitive care. More... »
