ABSTRACT

In 10–18% of cases, fractures of the humeral shaft are complicated by primary palsy of the radial nerve. Secondary iatrogenic lesions of the radial nerve occur in 3% of cases when internal fixation is achieved with an intramedullary nail, and in 15.5% after open reduction and internal fixation with a plate. In the vast majority of cases (80%), complete recovery of the radial nerve is reported. Vascular lesions affecting the radial artery are rare and require surgery with open local revision, vascular reconstruction and stabilization of the fracture with a plate. Medullary nailing is indicated as the primary treatment for simple, complex or pathological fractures of the humeral shaft or as a second procedure changed to following primary external fixation. While fractures involving the surgical collum of the humerus can be stabilized with a nail, fractures sited in the very distal part of the humerus are unsuitable for nailing. Fractures involving small fragments of the humeral head are characterized by an elevated complication rate. Because of the high incidence of lesions of the rotator cuff with antegrade nailing we are hesitant to use this technique, preferring retrograde nailing, which with careful adherence to the various indications and tailored postoperative treatment has proved to be a procedure that does not cause a high level of stress or discomfort for the patients and gives good functional results.
