ABSTRACT

Hepatische Metastasen neuroendokriner Tumoren stellen einen negativen prognostischen Faktor sowohl für die Überlebenszeit als auch die Lebensqualität der Patienten dar. Die Rationale der lokalen Behandlung der hepatischen Filiae ergibt sich aus der häufig auf die Leber beschränkten Metastasierung oder durch medikamentös nicht beherrschbare Symptome der neuroendokrinen Lebermetastasen. Als Alternative zur chirurgischen Resektion der Metastasen wurden eine Reihe von lokalen Therapieverfahren entwickelt. Zu den etablierten Verfahren zählen die Embolisation und Chemoembolisation, die sich wegen der Hypervaskularisation und hepatisch-arteriellen Gefäßversorgung anbieten, und die perkutane Alkoholinjektion. Neuere Verfahren sind die Radiofrequenztherapie, die laserinduzierte interstitielle Tumortherapie und die Kryotherapie. Nachfolgend werden die einzelnen Verfahren, ihre Indikationen, Ergebnisse und Komplikationen hinsichtlich der Behandlung von Lebermetastasen neuroendokriner Tumoren vorgestellt. More... »
