ABSTRACT

Hippocampal sclerosis denotes a neuropathological picture consisting of near complete nerve cell loss and gliosis in different sectors of the hippocampus. It occurs as a morphological feature in five clinical and neuropathological entities. In young patients, hippocampal sclerosis may be encountered in mesial temporal lobe epilepsy. Much more frequently, however, it occurs as part of the pathophysiology of neurodegenerative diseases in the old and oldest old. Major causes are frontotemporal lobar degeneration, non-Alzheimer tauopathies and most frequently, the recently described entity limbic age-related TDP-43 encephalopathy (LATE). This condition probably accounts for a considerable proportion of all degenerative dementias in high and very high age that are nowadays diagnosed as Alzheimer type dementia. Clinical data on this condition are yet scarce. In view of its occurrence in the most rapidly increasing age group of our population, it will be an important differential diagnosis in the future.
