ABSTRACT

Der kathetergestützte Aortenklappenersatz als Behandlungsoption der hochgradigen Aortenklappenstenose hat im letzten Jahrzehnt im Sinne einer disruptiven Technologie eine beeindruckende Entwicklung durchgemacht. Er stellt mittlerweile die Therapie der 1. Wahl für inoperable Patienten und Patienten mit hohem operativen Risiko dar. Hierbei nimmt der transfemorale Zugang den wichtigsten Stellenwert ein. Dennoch haben alternative Zugangswege im Rahmen entsprechender Indikationen nach wie vor entscheidende Bedeutung. Die vorliegende Übersichtsarbeit soll wichtige praktische Aspekte der unterschiedlichen Zugangswege des kathetergestützten Aortenklappenersatzes näherbringen. Literaturübersicht sowie bildliche Veranschaulichung wichtiger theoretischer und praktischer Aspekte der unterschiedlichen Zugangswege. Die Beherrschung interventioneller Techniken zur Durchführung eines kathetergestützten Aortenklappenersatzes, unabhängig vom Zugang, bildet einen integralen Bestandteil des Spektrums von Herzchirurgen. Hierbei sind nicht nur theoretische, sondern v. a. auch praktische Fertigkeiten von entscheidender Bedeutung für die professionelle Patientenbetreuung. More... »
