ABSTRACT

Die Arthroskopie ist bei der Behandlung proximaler Humerusfrakturen ein inzwischen fest etabliertes Verfahren. Isolierte und kombinierte Frakturen der Tubercula können mittels diverser Techniken arthroskopisch refixiert werden. Zudem ist die Nagelosteosynthese vorwiegend subkapitaler proximaler Humerusfrakturen arthroskopisch gestützt möglich. Neben den bekannten Vorteilen minimal-invasiver Techniken können zusätzlich die Frakturstellung und Implantatlage optimiert und kontrolliert werden. Relevante, bei den offenen Verfahren übersehene, intraartikuläre Begleitverletzungen werden bei den arthroskopischen Techniken diagnostiziert und therapiert. Auch bei der Behandlung von Folgezuständen proximaler Humerusfrakturen hat sich die Arthroskopie in den vergangenen Jahren als vorteilhaft erwiesen. Dieser Beitrag stellt die aktuellen arthroskopischen Versorgungsmöglichkeiten am proximalen Humerus dar und schildert Operationstechniken, Vorteile gegenüber offenen Verfahren sowie erste Ergebnisse.
