ABSTRACT

Aktuell stehen mehr als 20 für die Behandlung von Epilepsien zugelassene Substanzen zur Verfügung. Damit verdoppelte sich die Anzahl an Antiepileptika (AE) in den letzten 20 Jahren. Nicht alle AE sind im Kindesalter geprüft und zugelassen. Diese Medikamente werden nicht selten im Rahmen individueller Heilversuche als Reservemittel bei therapeutisch schwierigen Verläufen eingesetzt. Von den bei Kindern zugelassenen neuen AE konnten sich 4 Substanzen etablieren: (retardiertes) Oxcarbazepin, Lamotrigin, Levetiracetam und Topiramat. Zudem stehen 2 Medikamente mit dem besonderen Zulassungsstatus „orphan drug“ zur Verfügung. Diese werden zwar vergleichsweise selten eingesetzt, sind aber für die Anwendung bei Patienten mit Lennox-Gastaut- oder Dravet-Syndrom interessante und vielmals wirksame Substanzen. Voraussetzung für einen erfolgreichen Einsatz sowohl von alten als auch von neuen AE ist die Kenntnis ihres Wirksamkeits- und Nebenwirkungsspektrums einschließlich des Wechselwirkungspotenzials.
