ABSTRACT

La tiroidite post-partum (TPP), una patologia a patogenesi immunitaria che si manifesta nel primo anno dopo il parto, è probabilmente la malattia endocrina più frequente nel sesso femminile (dall’1, 1 al 16,7%). Presenta un decorso clinico che, nella forma classica, è caratterizzato da tireotossicosi distruttiva seguita da ipotiroidismo. I sintomi della tireotossicosi sono di solito modesti e difficili da riconoscere; nel 33% dei casi la tireotossicosi è silente. L’ipotiroidismo è talora più sintomatico, ma le sue manifestazioni possono essere confuse con i “normali malesseri” del puerperio. Nella maggior parte dei casi la funzione tiroidea si normalizza spontaneamente entro 12 mesi dal parto, ma le donne con TPP hanno un rischio elevato di sviluppare ipotiroidismo permanente nei successivi 5–10 anni. L’argomento dello screening per la TPP è stato oggetto negli ultimi anni di un ampio dibattito in letteratura, anche se al momento attuale non è stato raggiunto un consenso.
