ABSTRACTGrundlagen: Primäre intestinale Non-Hodgkin-Lymphome sind eine eigenständige Entität intestinaler Erkrankungen. 58% aller Operationen erfolgen unter Notfallbedingungen. Das klinische und therapeutische Dilemma besteht in der reaktiven Situation der Therapeuten. Methodik: In der prospektiv angelegten Studie „Intestinale Non-Hodgkin-Lymphome“ werden Daten zur Epidemiologie, Diagnostik, Histopathologie und multimodalen Therapie gewertet. Ergebnisse: Es wurden 26 Patienten mit einem intestinalen Non-Hodgkin-Lymphom protokollgerecht erfaßt und ausgewertet. Die Histopathologie ergab 14 hochmaligne, 4 niedrigmaligne mit hochmalignen Anteilen, 5 niedrigmaligne und 3 andere intestinale Non-Hodgkin-Lymphome. Bei 15 von 26 Patienten (58%) wurde die Diagnose nach einer Notlaparotomie am Operationsresektat gestellt. Durch Sonographie, Endosonographie, Intestinoskopie, Enteroklysma nach Sellink, CT von Thorax, Abdomen und Becken sowie durch Knochenmarkspunktion konnte in 42% präoperativ das Stadium entsprechend erfaßt werden, wie es durch ein regionales sowie juxtaregionales Lymphknotensampling und eine Leberbiopsie intraoperativ gesichert wurde. Schlußfolgerungen: Die hohe Rate inadäquater Therapieansätze des intestinalen Non-Hodgkin-Lymphoms ist durch die hohe Inzidenz der Notlaparotomien bedingt. Therapieentscheidend ist in allen Fällen ein adäquates Staging auch unter Notfallbedingungen. Nur die spezielle Kenntnis der Entität des intestinalen Non-Hodgkin-Lymphoms wird intraoperativ die Weichen zu der erforderlichen stadienadaptierten Therapiemultimodalität — Operation / Radiatio / Chemotherapie — stellen können.
