ABSTRACT

Die Einführung des CO2-Lasers in die endolaryngeale Mikrochirurgie hat zur Entwicklung spezifischer Techniken für die Stimmritzenerweiterung bei beidseitiger Rekurrensparese geführt. Die Resektion des Processus vocalis und des Aryknorpels eignet sich als laserspezifischer Minimaleingriff besonders für nichttracheotomierte Patienten mit behandlungsbedürftiger Dyspnoe und guter Stimmqualität. In einer retrospektiven Studie über den Zeitraum von 1983 bis 1994 wurden die funktionellen Ergebnisse bei 84 Patienten mit endolaryngealer Processus vocalis-Resektion mit dem CO2-Laser analysiert. Nach den vorliegenden Ergebnissen führt dieses phonochirurgisch orientierte Vorgehen zu einer signifikanten Verbesserung der Atemtraktswiderstände bei weitgehender Erhaltung der Larynxstrukturen sowie der Stimmqualität. Als laserchirurgischer Minimaleingriff kann die Processus vocalis-Resektion in der Mehrzahl eingreifendere chirurgische Maßnahmen der Glottiserweiterung ersetzen. Die endolaryngeale Glottiserweiterung in Form der Processus vocalis-Resektion, jeweils der individuellen Anatomie des Larynx angepaßt, kann sowohl gezielt vor chirurgischen Rezidiveingriffen an der Schilddrüse, und/oder an der Trachea als auch bei beidseitigen permanenten Rekurrensparesen mit Erfolg eingesetzt werden.
