ABSTRACT

Summary1. The current knowledge of histological, histochemical and electronmicroscopic studies of the conduction system are discussed and the normal blood supply of this system is described.2. The types of complete interruption of the AV-transmission are classified etiologically into experimental, congenital, traumatic, surgical and acquired non-surgical heart block. The most frequent heart block is the acquired non-surgical heart block. Complete heart block is most frequently found between the fifth and seventh decade, and occurs more often in men than in women.3. Our own studies and reports of the literature indicate that coronary sclerosis, especially arteriosclerosis of the intramural branches of the coronary arteries is the predominant lesion in the development of complete AV-block. Examination of the conduction system in such cases revealed diffuse and focal fibrosis of the AV-node, the bundle ofHis and of both bundle branches. The observations in the conduction system have been supplemented by planimetric measurements of the connective tissue of the myocardium using the integrational table (Leitz). It was found that in complete heart block there is a significant increase of the connective tissue in the myocardium of both the right and left ventricle and in the septum indicating disturbances of the coronary blood supply.4. The interpretation ofLenègre andZoob that 50–60% of all cases of complete heart block are due to idiopathic, primary, essential or so-called primitive degeneration of the conduction system is not confirmed by our studies. We have only encountered one such case among 15 cases carefully studied in which the heart of a 22-year-old woman showed fibrosis limited to the conduction system and in which there were no abnormalities of the myocardium or coronary vessels.5. Other causes of acquired complete heart block are described such as inflammatory diseases of the myocardium and endocardium, calcification of the anulus fibrosus, Chagas cardiopathy, osteitis deformans, Hodgkin's disease and tumors of the heart.
