ABSTRACT

The coincidence of hyperlipoproteinemia and arteriosclerosis is an established fact. Diet and medication are unable to lower pathologic levels of blood lipid by more than 25%. Partial ileum bypass surgery as done by H. Buchwald, inactivating the distal third of the small intestine, effectively influences the lipid metabolism. The following results were obtained from 15 patients who were operated upon: Seven patients with phenotype IIa, and seven patients with phenotype IIb (Frederikson), returned to normal patterns 1 year after surgery. Total cholesterin levels were reduced by 41% on average (P<0.001). Triglycerides showed considerable variations. Lipid electrophoresis revealed that lowered cholesterin levels were mostly due to a decrease of the sclerosis-promoting fraction of LDL. Whether the operation results in accelerating LDL catabolism, or in impairing LDL synthesis, is still being investigated. Genuine complications were not observed. Increased frequency of stools (6–8 per day) returned to normal within 6 months. Substitution of vitamin B 12 was necessary every 3 months. More... »
