ABSTRACT

Out of more than 3/12 thousand arterial reconstructions performed in the Heidelberg Surgical Clinic (1959-31.12.1971) 1332 were aimed at chronic occlusions of the femoro-popliteal segment.Whereas at first semiclosed thrombendarteriectomy was preferred as a simple and rapid procedure, this is now carried out only under strict preoperative criteria and under intraoperative control of the disobliterated vessel. Late angiographic follow-ups, showing alarming progression of arteriosclerosis in the stripped artery, and a recurrency rate of 44% after 4 years in stage III and IV patients, confirm the superiority of the alternative procedure: femoro-popliteal vein by-pass (recurrency rate for comparable cases: 25%).
