ABSTRACT

Following distal splenorenal shunts according to Warren 36 patients were examined up to 5 years postoperatively. Hospital lethality was 8 % in patients of Child class A and B; operative lethality was 0 %; shunt occlusion rate was 3.1 %; late recurrence of hemorrhage was 3.1 %; postoperative encephalopathy was 4.7 %. Hemodynamic evaluation 3–5 years postoperatively showed new formation of collaterals between the portal vein and the gastrosplenic shunt system associated with pressure drop in the portal system. Therefore, the hemodynamic basic principle of the Warren shunt is not stable according to long-time observation. More... »
