ABSTRACT

Gut hormone changes after dietary loading were measured in experimental dog models to determine the worth of preserving the pyloric ring in a pancreaticoduodenectomy. The dogs were divided into two groups: one received a pylorus-preserving pancreaticoduodenectomy (PPPD group,n = 10) in which both the entire stomach and the pyloric ring were preserved; the other group received a pancreaticoduodenectomy (PD group,n = 10) in which the entire stomach, but not the pyloric ring, was preserved. After surgery, the gut hormone values in both groups were compared with the preoperative gut hormone values in a control group(n = 20) that consisted of these same animals prior to surgery. The PPPD and the control group showed a similar response to dietary loading, with both the integrated gastrin response (IGR) and integrated secretin response (ISR) being roughly analogous. In contrast, the PD group showed minor changes in serum gastrin levels after dietary loading, and their IGR response was significantly lower(P < 0.05) than that of the PPPD and control groups. In the PD group, serum secretin levels were elevated 5 and 10 min postprandially, but then decreased. The ISR was significantly lower(P < 0.05) in the PD than in the PPPD and control groups. These results indicated that preservation of the pyloric ring in a pancreati-coduodenectomy is of great merit, since it aids in maintaining a better postoperative gut hormone response. More... »
