ABSTRACT

After the onset of an event inducing ischemic myocardial necrosis, the heart wall undergoes a stereotyped series of inflammatory and reparative responses. Two distinct types of ischemic myocardial injury can be recognized by pathologic study; each by its presence is indicative of the pathophysiologic event that has occurred. Coagulation necrosis results from persistent obstruction to blood flow. Contraction band necrosis occurs when coronary reflow follows a period of no perfusion. The overlying endocardium also participates in the response to an infarct and its reaction may be markedly altered depending on the presence or absence of mural thrombus. In individual patients, there may be very different gross topographic changes in infarcted segments of the heart wall, depending on the extent of coronary artery disease, the nature of the coronary occlusion, the size of the myocardial infarct, and the character of the endocardial response. Thus, despite a predictable course of the separate pathological processes in ischemic heart disease, the analysis of the specific case may be very difficult and requires detailed postmortem angiographic, gross, and histologic study of the heart.
