ABSTRACT

We have experienced 328 pancreatoduodenectomies during a 23-year period from January 1972 to December 1994. Recently, about 50 of these operations have been performed annually, and the trend is increasing. The most common diagnosis of resectable cases was ampulla of Vater cancer, followed by distal common bile duct, pancreas head, and duodenal cancer. The overall operative morbidity and mortality were 37.5% and 3.4%, respectively, and the rates are continuing to decrease. In studies conducted to determine the operative methods that can be performed with the lowest morbidity and mortality, no one method has proved to be the best, and the most important thing may be strict adherence to basic surgical principles and the patient’s general condition. The indications for pancreatoduodenectomy should be refined when more experience is gained. More... »
