ABSTRACT

Our case is a 15-year-old high school girl. At the age of 8 years, she first noticed a splayfoot deformity, which, began causing pain at the 5th metatarsophalangeal (MTP) joint at the age 12. Since then, she has had difficulty finding comfortable shoes. The symptoms became worse and the only shoes she could wear other than sneakers were lace-up shoes (Fig. 1). When she was seen at our hospital, marked splayfoot deformity was found bilaterally. Although weight-bearing accentuated the deformity, there was no hallux valgus or flatfoot deformity noted. A tailor’s bunion had formed on the lateral side of each 5th MTP joint which was very tender (Fig. 2). On weight-bearing roentgenogram, the 1st and 2nd intermetatarsal angles appeared within the normal range (12° on the right foot and 10° on the left). The obvious abnormalities noticed were the increased intermetatarsal angles between the 4th and 5th metatarsals and the 1st and 5th (Fig. 3). Both angles were increased on weight-bearing.
