ABSTRACT

Crohn’s disease is a long-lasting intractable inflammatory disorder of the large or small intestine with unknown etiology. Investigations suggest that combined genetic and environmental factors such as infectious agents or intraluminal antigens play important roles in the pathogenesis of the disease. Among them, we have proved that impaired immune responses in noncaseating granuloma, which has been thought to be an important histopathologic feature of the disease, play a crucial role in initiating candidate pathognomonic antigens. Immunohistochemical analysis of infiltrated cells around a granuloma revealed that class II major histocompatibility complex molecules and co-stimulatory molecules such as CD80/86 were up-regulated in the granuloma cells of macrophage lineage origin. Such granuloma cells often made close contact with CD28-positive T cells with either CD4 or CD8. These granulomas were often located in the luminal spaces, of which endothelial cells coating the lumen did not express vascular endothelial cell markers such as CD36 or von Willebrand factor. Electron microscopy showed that those cells were lymphatic endothelial cells. These results strongly suggest that noncaseating granulomas of Crohn’s disease are formed close to lymphatic vessels, and antigenic stimuli through the lymphatic system may be important in the pathogenesis of Crohn’s disease.
