ABSTRACT

In an attempt to evaluate the pathophysiology of arteriovenous malformations (AVMs) and the prognostic factors in patients undergoing surgery, pre-operative cerebral blood flow (CBF) measurements were performed in 14 patients: the examinations were repeated after acetazolamide infusion (1g i.v.) in order to investigate residual vascular reserve and vasoreactivity for regional and hemispheric assessment. The 133-Xe inhalation technique was used and the data processed through the M2 model elaborated by Prohovnik. Fourteen patients were studied; the mean volume was 26 cm3; 4 patients experienced post-operative hyperemic complications. Mean resting CBF values (54 m1/100 g/min) showed a variable increase after acetazolamide (average 35%, ranging from 5 to 52%), higher in the patients affected by hyperemic complications (41 vs. 26%). Moreover: a) regional activated-CBF showed a correlation with the baseline values, with higher response in patients with lower values; b) hemispheric activated-CBF was influenced by AVM volume, showing lower increase in larger AVMs. A common mechanism has been suggested to explain these unexpected features.
