ABSTRACTDie Resektabilität von hepatozellulären Karzinomen ist bei begleitender Leberzirrhose durch die eingeschränkte Leberfunktion limitiert. Mit der Transkatheter-Arteriellen-(Chemo)Embolisation (TAE) ist ein Verfahren verfügbar, mit dem bei Irresektabilität eine Palliation und gelegentlich auch eine Regression des Tumors ermöglicht wird [1, 3, 4, 6, 7, 10–16]. Allerdings ist bei Chemoembolisation wie auch bereits bei einfacher Embolisation ohne zusätzliche Gabe eines Chemotherapeutikums eine Leberschädigung mit Funktionseinschränkung zu beobachten [3, 5, 8, 9, 10, 16]. Diese Leberschädigung kann ischämiebedingt sein. Sie ist möglicherweise aber auch eine Folge der Freisetzung von nekrotischem Material aus den embolisierten Tumorarealen. Da für das Überleben von Patienten mit Leberzirrhose bereits eine geringe Funktionseinschränkung der Leber von entscheidender Bedeutung sein kann, haben wir in der vorliegenden Studie die Auswirkungen der Chemoembolisation auf die Leberfunktion von Patienten mit hepatozellulärem Karzinom bei bestehender Leberzirrhose untersucht.
