ABSTRACTDie große Mehrzahl der hepatozellulären Karzinome (HCC) wird, aufgrund der großen Fortschritte der bildgebenden Verfahren (CT, Sonographie), inzwischen klinisch diagnostiziert und stellt nicht mehr einen Zufallsbefund im Rahmen autoptischer Untersuchungen dar. Dennoch ist die überwiegende Anzahl der Patienten mit hepatozellulärem Karzinom zum Zeitpunkt der Diagnosestellung inoperabel [3]. Aus diesem Grund ergibt sich auch häufiger die Situation, daß Patienten mit bekanntem hepatozellulären Karzinom und akuten Komplikationen dieses Grundleidens oder Komplikationen eines koinzidenten anderen Leidens dem Chirurgen vorgestellt werden. In solchen Situationen (Ruptur des Leberkarzinoms, Tumorblutung, iatrogene Komplikationen, extrahepatische akute Erkrankungen) ist eine sorgfältige Abwägung des Gewinns einer notfallmäßig durchgeführten chirurgischen Intervention im Verhältnis zu Zustand und Prognose des Patienten vorzunehmen. Um der Frage nach der Häufigkeit, der Prognose und dem klinischen Verlauf der beschriebenen Patientengruppe nachzugehen, wurde das Krankengut der Chirurgischen Universitätsklinik Tübingen einer retrospektiven Analyse unterzogen.
