ABSTRACTDa die konventionelle Röntgendiagnostik eine ausreichende Beurteilung von kindlichen epiphysären Frakturen häufig nicht zuläßt, ist eine genauere Analyse durch Schnittbild-verfahren empfehlenswert. In einer prospektiven Studie sollte ermittelt werden, ob durch den MRT-Befund eine Beeinflussung der Frakturklassifikation und des therapeutischen Vorgehens resultieren würde. Untersucht wurden 36 Patienten mit röntgenologisch festgestellter oder vermuteter wachstumsfugenbeteiligender Fraktur unterschiedlicher Lokalisation. Bei 67% der Patienten führte die MRT zu einer Änderung der Klassifikation. In der Hälfte der Fälle resultierte durch die MRT eine Änderung der Therapie. Dabei wurde in 8 Fällen aufgrund des MRT-Befundes die konservative Therapie der Fraktur modifiziert (minor change) und bei weiteren 10 Patienten anhand des MRT-Befundes die Indikationsstellung zur Operation geändert (major change). Zusätzlich zeigte die MRT in 6 Fällen relevante ligamentäre Verletzungen.
