ABSTRACT

Introduction: Bone quality and the osseous structure are of decisive importance for both origination of fractures and fracture treatment. Knowledge of osseous microstructures enables statements about fracture treatment and shows the limits of implants. Furthermore, these analyses could make an important contribution to the development of new implants. Material and Methods: In 24 freshly harvested human cadaver humeri histomorphometric analysis and indentation testing were performed. The median age was 46 years (range 34–76) in the male group (n=11) and 69 years (range 46–90) in the female group (n=13). According to its radius the humeral head was divided into four equal horizontal levels. Within the four cutting planes used for the mechanical testing, five regions of interest (ROIs), anterior, posterior, lateral, medial and central, were defined. Simultaneously, histomorphometric analysis with the evaluation of two structural parameters (TV/BV, Nd.N) and trabecular orientation was performed in the same regions. Results: Significant differences regarding bone quality and osseous structure in the differing regions were found in the histomorphometric analysis, as well as in the indentation testing. The medial and dorsal aspects of the proximal humeral head were found to be the areas of highest bone strength. The structural parameters and biomechanical properties also showed region-related characteristics. Conclusion: The findings about region-related microstructures and bone strength supply us with the basics for the development of new implants on the proximal humerus. According to our findings, the positioning of implants in proximal humerus fractures should be carried out in the areas of highest bone strength. More... »
