ABSTRACT

The objective of this chapter is to describe the technique of anterior C1–C2 fixation or in combination with anterior fusion. It is indicated in elderly patients with type II odontoid fracture and accompanied atlanto-odontoid osteoarthritis and is regularly performed in combination with anterior odontoid screw osteosynthesis. Preoperatively, accurate preparation and planning are necessary, such as exclusion of high-riding A. vertebralis. The operation is done in supine position. Closed anatomic reduction is mandatory. The surgical approach is according to Smith-Peterson. Drilling can be performed using 2 mm K-wires. The entrance point is direct lateral to the tips of the dens screw(s) at the anterior endplate of C2. The direction is tilted 20° to 25° laterally and 30° to 40° dorsally. Cannulated screws with 3.5/4.0 mm diameters and short thread are recommended. In conclusion, anterior C1–C2 fixation is a safe surgical technique with a low approach-related morbidity leading to promising 1-year results in geriatric patients with type II odontoid fractures and C1–C2 osteoarthritis.
