ABSTRACT

Atrial septal defects, the second most frequent congenital cardiac malformations, cause left to right shunting at atrial level. Enlargement of the right atrium, ventricle and pulmonary artery can be well displayed by 2D echocardiography. In infants and children, the description of location and extension of atrial septal defects is greatly facilitated by the application of the subcostal views. This applies specifically for the visualization of sinus venosus defects. Suspected defects in the atrial septum should always be confirmed in more than one plane by the application of colour Doppler echocardiography. In patients with large defect, pulsed wave Doppler reveals increased flow velocities across the tricuspid and pulmonary valve. Pulmonary hypertension is encountered only in a minority of cases.
