ABSTRACT

Serrated polyposis syndrome (SPS), formerly known as hyperplastic polyposis syndrome, is an uncommon disease characterized by the presence of multiple serrated polyps throughout the colorectum. Until now, its genetics, molecular, and clinical characteristics continue to be unknown. This syndrome could be considered as the paradigm of the serrated pathway of carcinogenesis. Diagnosis is made by the fulfillment of any of the World Health Organization (WHO) criteria, recently redefined: (1) At least five serrated polyps proximal to the sigmoid colon, with two or more of these being larger than 10 mm; (2) Any number of serrated polyps proximal to the sigmoid colon in an individual who has a first-degree relative with SPS, and (3) More than 20 serrated polyps of any size, distributed throughout the colon.In this chapter we will review the clinical and epidemiological characteristics of this syndrome, the main hallmarks of the serrated pathway of carcinogenesis, and the risk of cancer and recommendations for surveillance for SPS patients and their relatives.
