ABSTRACT

This chapter discusses the preoperative considerations of a patient undergoing thoracic surgery. Physiological effects, cardiac risk assessment, and preoperative investigations are summarized, along with strategies for mitigating cardiopulmonary adverse events. One of the most important skills a thoracic surgeon must master is the determination of operability, namely, a patient’s ability to tolerate thoracic surgery—most commonly pulmonary resection. Thoracic surgical patients tend to have reduced baseline cardiopulmonary function due to cigarette smoking, COPD, atherosclerotic cardiovascular disease, and possibly hypertension, diabetes and advanced age, putting them at significant risk of perioperative morbidity and mortality.
