ABSTRACT

Transjugular intrahepatic portosystemic shunt (TIPS) placement is an established and vital treatment option for relieving complications of portal hypertension, including gastroesophageal variceal bleeding which has been unsuccessfully controlled medically. Acute variceal bleeding can be a life-threatening event, with immediate mortality ranging from 5 to 8 % and reaching up to 20 % within 6 weeks. Medical therapy, including vasoactive drugs, prophylactic antibiotics, and endoscopic intervention, is the recommended standard of care for patients with acute variceal bleeding, but may result in treatment failure in 10–15 % of patients. Use of TIPS for treatment of variceal hemorrhage has been validated in large clinical studies, and early TIPS application in acute variceal hemorrhage, particularly with a covered stent-graft, has been shown to be associated with reductions in failure to control bleeding, rebleeding, and mortality. The purpose of this chapter is to review the interventional approach to TIPS placement, focusing on the treatment of medically refractory variceal hemorrhage. More... »
